AHCM Advisory Board

History

In 2017, a group of community organizations and clinical providers across Western Colorado agreed that we, in Western Colorado, have a shared responsibility and opportunity to improve the health of people and communities across Western Colorado.  The group developed a charter and vision that by supporting and empowering our entire community, especially those members who may have additional resource needs, we are able to improve the health of the entire community.   

Mission

We are partners in creating a more effective network, a stronger culture, and a more seamless system for supporting the social, emotional, physical and economic health of Western Coloradoans. This involves:

  • A coordinated screening strategy to identify what supports people need
  • Strong connections and handoffs between clinical and community partners
  • Policies and infrastructures that support collaborative approaches to health
  • Empowering communities to transform how health is promoted.

Role of the Advisory Board

The AHCM advisory board is responsible for ensuring that we continue to focus on this big picture vision of improving the health of Western Coloradans by addressing social determinants of health. One concrete step in that effort is participation in the Accountable Health Communities Model. Both the big picture vision and AHCM require shared ownership between community organizations and clinical partners.

The Logistics

The AHCM Advisory Board meets in person quarterly in Grand Junction. If you are interested in participating in the Advisory Board, please contact Kathryn Jantz.
