Community Programs

Across the Western Slope of Colorado, RHMP supports programs to build community and support providers as they improve care for individual patients and the larger community.

Health First Colorado’s Accountable Care Collaborative (ACC) is a central part of Medicaid reform.  RMHP serves as the Regional Accountable Entity (RAE) in Colorado's Region 1, which includes 21 counties on the Western Slope and Larimer County.  RMHP Prime is a related progbram intended to improve the health of individuals and the community while reducing costs. Both of these community-based approaches brings together payment reform, population health management and whole-person care.

RMHP also sponsors practice transformation, working with practices to support quality improvement and skills development. Practice transformation includes the programs linked below.

Rocky Mountain Health Plans' Practice Transformation Team is currently taking applications for the following programs to begin in January 2019: Foundations, Foundations Specialty, Masters 1, Masters 2, Patient-Centered Medical Home (PCMH), and Patient-Centered Specialty Practice (PCSP).  Learn more about why your practice should participate here.
