The Community Health Action Coalition (CHAC) is the coordinating body for communication, collaboration, education and other health-related activities in La Plata County. It functions as an open, inclusive model to facilitate collaboration, relying significantly on volunteers with few paid staff. Anyone may participate, but it encourages everyone to become paying members. CHAC works with all community members, but it has a particular focus on the underserved residents of La Plata.

MISSION: Community Health Action Coalition (CHAC) is the coordinating body for communication, collaboration, education and other health related activities in La Plata County. We advocate for the prevention of disease and disability, healthy lifestyles and integrated health services that are accessible and affordable to everyone.
VALUES: We believe in person-centered health and wellness that honors the whole human within the context of family and community, including mind, body, emotions and spirit.
HISTORY: In 2001, CHAC emerged from a breakout session during the La Plata County Community Summit. More than 50 people attended the “Access to Healthcare” breakout session, including community leaders, elected officials, health care providers, non-profit representatives and many interested citizens. Together, these participants envisioned that a broad coalition dedicated to improving access to health care was needed. In recent years, this focus has expanded to encompass prevention, behavioral and oral health. (A more detail history is available at www.chaclaplata.org/history.asp.)
