The Accountable Care Collaborative is designed to help Medicaid clients connect with doctors, other health care providers and local services and supports. Care coordinators play a critical role in this system.
Rocky Mountain Health Plans also supports providers with care coordination resources, in partnership with multi-disciplinary local care coordination teams. These care coordinators work collaboratively with medical and behavioral health providers to provide appropriate services for members. Through care coordination, physical and behavioral health needs of members are identified and services are facilitated and coordinated with all service providers, individual members and family, if appropriate and authorized by the member.
