The Practice Transformation Team falls within the department of Clinical Program Development and Evaluation at RMHP. The Practice Transformation Staff bring a variety of background expertise as it relates to quality improvement in the primary care setting. This staff is available to support the work at the practice site in a face-to-face setting that is supplemented by webinars and day long learning collaborative events in which practices have the opportunity for peer-to-peer learning. Leadership staff is comprised of the following:
Lori Stephenson, RN is the Director of Clinical Program Development and Evaluation holds degrees in nursing and organizational management. Mrs. Stephenson has been with RMHP for 25 years. She held the position of Quality Improvement Director for 17 years and was responsible for RMHP’s Quality Improvement activities as well as Continuing Medical Education projects, Credentialing, the Provider Review process and the HEDIS/CAHPs reporting activities. She currently directs the Practice Transformation and Data Analysis Team.
Kristi Hall, MA, BS, CCM is the Program Manager of the Practice Transformation Team. She holds a Bachelor of Science degree in Human Development and Family Studies (with a concentration in Adulthood) and a Master’s degree in Gerontology. Mrs. Hall has experience in behavioral health, long term care as a Social Services Director, and three years as a Nursing Home Administrator. She also worked as an independent geriatric case manager and holds a certification in case management. She began her career at RMHP in 2014 as a Quality Improvement Advisor working with the practices in the southwestern part of the state.
Kim Brown, MHA, BS, RT(R), CPEHR is the Manager of Clinical Analytics and Informatics. She holds a Bachelor’s Degree in Medical Imaging and a Masters Degree in Healthcare Administration. She is a certified professional in Health Information Management Systems. Mrs. Brown previously worked in the field of healthcare IT and began her career at RMHP in 2013 as a Quality Improvement Advisor. In her current role as Manager she works collaboratively with her team to support practices with health information technology needs including compiling and analyzing data. is the Supervisor of the Clinical Informaticists. She holds a Bachelor’s Degree in Medical Imaging and a Master’s Degree in Healthcare Administration. She is a certified professional in Electronic Health Records. Mrs. Brown previously worked in the field of healthcare IT and began her career at RMHP in 2013 as A Quality Improvement Advisor. In her current role as a Clinical Informaticist she works collaboratively to support practices with health information technology needs including compiling and analyzing data.
The Program Education and Communication Coordinator maintains coordination and implementation of Practice Transformation program educational offerings such as Leaning Collaboratives, webinars and communications with program participants
Mindy Patton, BS holds a Bachelor of Science degree in Marketing Management, Business and Hospitality. Mrs. Patton came to Rocky from the transportation industry and has 10 years of experience in leadership and continuous improvement in a rapidly evolving market.
The Integrated Behavioral Health Advisor role is filled by a licensed mental health professional who has training and experience in delivering behavioral health services within primary care. The IBHA shares evidence-based recommendations with practices pertaining to clinical assessment and brief interventions and strategizes with practices how to build sustainable operational systems to support the integration of physical and behavioral healthcare.
Alex Hulst, PhD, LMFT holds a PhD in Marriage and Family Therapy with a Certificate of Advanced Specialization in Medical Family Therapy. She has over five years of clinical experience providing therapy for individuals, couples, and families in various primary and specialty care settings, including pediatrics. Dr. Schmidt’s background includes training resident physicians and mental health clinicians in integrated behavioral health practices, leading multidisciplinary consultation groups in care management, and assisting with grant management supporting integrated behavioral health in rural and smaller practices in western Colorado.
Quality Improvement Advisors (QIAs) are specialists with a wide variety of background expertise related to practice transformation and quality improvement. QIAs provide support to the practice in workflow analysis, efficiency, effectiveness, and in teaching quality improvement techniques.
Mary Beckner holds a degree in Business Management. Mrs. Beckner came to RMHP from the Seattle Cancer Care Alliance where she was introduced to the Toyota Way, and assisted in implementing Lean methodologies to the organization using continuous improvement.
Anna Messinger, MHA, PCMH CCE holds a Bachelor of Science degree in Psychology and Business Administration and a Master’s Degree in Healthcare Administration. Ms. Messinger came to RMHP from the University of Colorado Cancer Center, where she worked primarily in the realm of financial administration for Phase I clinical trials. She is a Certified Content Expert in PCMH.
Barbara Bishop, BA, MA holds a Bachelor of Arts in Anthropology and a Master of Arts in Historical Administration. She previously worked as a Customer Service Universal Representative for RMHP and has prior experience in a medical office setting.
Sara Jordan BS, MS holds a Bachelor of Science in Kinesiology and a Masters of Science – Exercise Science and Health Promotion. She previously worked as a Customer Service Universal Representative for RMHP and has prior experience coaching at the collegiate level.
Rae L. Sanchez, BHA, MSA holds a Bachelor of Science in Health Care Administration and a Master of Science in Administration. Rae is a certified medical coder through the AAPC. Rae has worked in various roles in healthcare over the last 18 years. Most recently, Rae has worked in medical practices in a variety of roles to include medical scribe, medical assistant, coder/biller, Clinic Administrator and Practice Manager.
Jared Ward, MSHA holds a Bachelor of Science degree in Psychology and a Master’s of Science in Healthcare Administration. Mr. Ward came to RMHP from Children’s Hospital Colorado working in administration, primarily in customer service with occupational and physical therapy. He brings great knowledge working with Epic Systems.
Rachael Acosta Biller, MSHA, BS holds a Bachelor of Arts degree in Psychology and Kinesiology and a Master’s degree in Healthcare Administration. Mrs. Biller comes with 18+ years of practice and business management experience. Prior to coming to Rocky Mountain Health Plans, she gained her knowledge by working with Health One, Kaiser Permanente, and a few private, multi-specialty practices in the Denver area.
Clinical Informaticists (CI) are specialists in the field of health information technology, the use of EMRs, data collection and analysis. CIs can provide support to the practice through feedback on how to maximize the use of technology in the practice and through facilitation of the effective use of data through integration into quality improvement activities. CIs assist practices with optimizing health information technology and the use of EMRs and data mapping, data reports, report building, registries, support for MU, and face-to-face coaching on the meaning and application of practice’s data.
Jennifer Walterscheid, BS holds a Bachelor’s Degree in Computer Information Systems. She previously worked in the public health sector on software and EHR implementation and management, quality improvement, and program data and measurement. In her current role as a Clinical Informaticist, she works collaboratively with the Practice Transformation Team, to provide support that meets the needs of the practices and the team, including database management, reporting abilities, and data analysis.
Chelsea Watkins, MHA, BS, CHES has a Masters of Health Care Administration and Management with an emphasis in Finance and a Bachelor of Science in Health Promotion and Education with an emphasis in Community Health. She is a Certified Health Education Specialist. Her prior experience includes working in the public health sector as a Health Promotion Specialist on the health promotions and informatics team. Ms. Wells’ career at RMHP began as a QIA in 2016, and in her current role as a Clinical Informaticist, she serves practices in RMHP programs, SIM, and TCPi. Ms. Wells is also the TCPi-lead for the Practice Transformation Team.
Annie Schudy, BSN, BA, RN holds a Bachelor of Science in Nursing and a Bachelor of Arts with an emphasis in Speech, Language, Hearing Sciences and Disorders. Ms. Schudy joined RMHP in January 2018 as a Clinical Informaticist. In her current role she works collaboratively to support practices with health information technology needs including compiling and analyzing data and coaching practices on the meaning and application of the data. Her prior experience includes working for a health information exchange as a Clinical Account Manager where she worked with hospitals and practices in western Colorado to improve care through the application of health technology.
Bronte Smith, BS, MHA holds a Bachelor of Science in Kinesiology and finishing her Master of Healthcare Administration with a specialization in Human Resources. Her experience is primarily in specialty clinics with orthopedics, sports medicine, and rehabilitative/physical therapy services in various administrative roles as an office manager, billing, insurance verification, authorization, and claims responsibilities.
Doug Bolton, MSN, RN holds a Masters of Science in Nursing with an emphasis in Healthcare Informatics. His nursing experience is in hospital acute care, specifically cardiac critical care. His Informatics experience is in hospital EMR implementation, support, and optimization. He also has experience in EMR reporting and data analysis.
