Transforming Clincial Practice initiative (TCPi)

The Transforming Clinical Practices Imitative (TCPi) is a free, federally funded support network that will help you and your care team prepare for value-based payment and achieve the Quadruple Aim – improved care, reduce costs, improved patient experience, and restore the joy of practice. This 4 year program is open for clinicians in both specialty and primary care. Within the program, there are three primary drivers that will help guide your practice’s transformation:

  1. Person and Family-Centered Care Design
  2. Continuous, Data-Driven Quality Improvement
  3. Sustainable Business Operations

TCPi will help your team implement impactful changes to improve care for you patients, families, and communities by utilizing quality improvement tools and strategies, collaborating with similar practices, and access to resources, such as presentations, journal articles, toolkits and one on one coaching with a Practice Facilitator and a Clinical Health Information Technology Advisor (CHITA).

Program Deliverables:

  • Identify one clinician and one staff member to be “Practice Champions”
  • Collaborate in monthly virtual learning communities with other participating practices
  • Work with Practice Facilitator’s (PFs) and Clinical Health Information Technology Advisors (CHITAs) on implementing tests of change and monitor progress towards meeting your quality improvement goals
  • Attend two face-to-face regional collaborative learning sessions each year
  • Submit appropriate clinical quality measures to the Colorado data hub
  • Complete baseline and periodic practice assessments
