The practice transformation team works to develop, enhance and help implement skills, processes and infrastructure to support ongoing improvement and delivery of effective, efficient primary care.

We work with practices to:

  • Develop a functioning quality improvement team within the practice;
  • Implement proven models for delivering clinical guideline-driven care; and 
  • Develop accurate, timely and actionable reporting for quality improvement.

Support services we provide include:

  • Individualized quality improvement advisor (QIA) support in your practice;
  • Clinical systems advisor support; and
  • Free tools and resources, including CME credits available for participation in each Learning Collaborative, our sessions in which practices come together to learn from each other and share best practices, strategies and practice transformation techniques.

Because practice transformation enhances the effectiveness of patient care and population health, RMHP incentivizes practices to participate in these programs. Twice each year, we actively recruit primary care practices to participate in our program.
