What happens when the community gets behind community corrections?

Larimer County awarded its grand prize for innovation last week to a program that helps recovering addicts get needed medical care. The Community Corrections and Health Care Collaboration involves onsite assessments by paramedics from UC Health Northern Colorado. They pass along the results to Rocky Mountain Health Plans case managers. Rocky works with residents to enroll in Medicaid, if needed, and to connect with a primary care provider. The program helps reduce costly use of emergency room care.
