Foundations  is an introductory level course in which participants learn basic QI skills at the practice level to include Plan, Do, Study, Act  (PDSA) cycles, process mapping, and basic data use to improve/develop and implement skills, processes, and infrastructure to support ongoing improvement and the delivery of effective and efficient primary care.

Program Objectives:

  1. Develop functioning quality improvement team within the practice
  2. Implement proven models for delivering clinical guideline-driven care
  3. Develop accurate, timely and actionable reporting for quality improvement
  4. Empanel entire active patient population

Practice Deliverables:

  1. Create a practice quality improvement (QI) team that meets twice monthly to:
    1. Implement the Program Change Package
    2. Implement the Care Model
    3. Implement the Model for Improvement and Plan Do Study Act (PDSA) Cycles
  2. Meet with Quality Improvement Advisor and Clinical Informaticist on a regular basis
  3. Submit quarterly narrative reports to RMHP for the purpose of assessing progress by highlighting best practices and challenges
  4. Submit quarterly clinical quality measures to RMHP for entire patient population
  5. Participate in at least 2 Learning Collaboratives
  6. Share lessons learned and best practices, through informal, verbal sharing, at the Learning Collaborative Series
